Please speak with Pinnacle Pension Services if you have an Individual Medical or Dental Insurance Policy for you or your dependents. You may be able to participate in the Insurance premiums portion of the plan, if the policy qualifies. Please note that disability, life insurance or cancer polices are not eligible for reimbursement through this benefit plan.
Reimbursement will be available only for "qualifying medical care expenses". You are responsible for notifying the Company if you have reason to believe that any expense for which you have obtained reimbursement is not a qualifying expense. You are responsible for indemnifying and reimbursing the Company on demand for any liability it may incur for failure to withhold federal, state or local income tax or Social Security tax from any reimbursement you receive for a non-qualifying expense, up to the amount of additional tax actually owed by you.
